United Way Contributor Choice Plan
Card
One way to help is to make a contribution on the behalf of Verland through the United Way
Contributor Choice Plan Card. With your help, Verland will continue to make a difference
in people's lives.
Just follow these simple steps:
1. Ask your employer for a Donor Option Card
distributed by the United Way. You will also need a United Way Pledge Card.
2. Under the Donation Card Section (Option 2), designate Verland as your donor option
agency.
3. Fill out both sides of the card. Remember to check the box authorizing the
United Way to inform Verland of your generosity.
4. Return the completed United Way Pledge Card and the Contributor Choice Plan Card to your United Way worker.
